Designing Audit Trails for Agentic Healthcare AI
Agentic healthcare AI can retrieve records, call tools, update systems and delegate tasks. When an incident occurs, reconstructing the sequence of events is essential for debugging, safety review and accountability. Logging only the final answer is insufficient. What should an audit trail capture?

Agentic healthcare AI can retrieve records, call tools, update systems and delegate tasks. When an incident occurs, reconstructing the sequence of events is essential for debugging, safety review and accountability.
Logging only the final answer is insufficient.
For consequential workflows, consider recording the agent and model version, relevant input references, timestamps, applicable permissions, tool calls, execution results, approval decisions, human overrides and subsequent corrections.
Where possible, reference existing clinical records rather than unnecessarily copying sensitive patient data into logs.
An agent may recommend an action without executing it. It may also attempt an action that fails or execute an action that another system later reverses.
Record these as distinct events. A useful event model distinguishes the proposed action, authorisation decision, execution attempt, confirmed result and any later correction.
Use correlation IDs or equivalent workflow identifiers to connect related events across agents and services. Synchronised timestamps, versioned events and reliable status records can help reviewers understand the sequence of a multi-agent workflow.
Do not assume that an attempted tool call proves the downstream action succeeded. Capture the result or clearly record that confirmation remains unavailable.
Audit records can contain sensitive health information. Apply least-privilege access, retention controls, data minimisation and tamper-evident storage where appropriate. Ensure that administrative access and changes to the audit system are themselves governed.
Simulate failed tool calls, denied permissions, human overrides, duplicate actions and conflicting updates. Then ask an authorised reviewer to reconstruct what happened using the available records.
The objective is not to log everything indiscriminately. It is to capture sufficient trustworthy evidence to investigate consequential actions and establish whether the system followed its authorised boundaries.
Engineering principle: If a consequential AI action cannot be reconstructed, effective oversight becomes significantly harder.
If you are building healthcare agents, include audit-trail completeness and integrity in your deployment acceptance criteria.
