Healthcare AI agents are often evaluated by whether they execute tasks correctly. However, technical completion and real-world success are different properties.

Consider an agent that schedules a follow-up appointment. The scheduling operation may succeed, but the patient may never attend. Similarly, an agent may transmit a referral without confirming that the receiving service accepted it.

Developers should distinguish between action-level success and goal-level success.

Action-level monitoring verifies that individual operations occurred as intended. Goal-level monitoring evaluates whether the desired endpoint was achieved, using appropriate evidence and within the system's authorized scope.

A practical agent architecture should define measurable outcomes, track intermediate states, detect stalled workflows, and identify conditions requiring escalation. It should not claim clinical resolution merely because an API call or administrative operation succeeded.

Outcome evaluation also requires caution. Patient outcomes are influenced by many factors, so an observed improvement cannot automatically be attributed to the AI system.

The objective is to design agents that demonstrate meaningful follow-through while preserving clinical oversight and appropriate accountability.

Call to action: AI engineers and healthcare technology teams, how are you distinguishing task-level success from outcome-level success in your systems?
